The discounts are available through participating healthcare providers only. The DMPO does make available a list of all program providers which includes their name, city & state, and medical specialty prior to purchase, upon request. To check that your provider participates, simply locate providers on our website or call 1(877) 954-4502. Since there is no paperwork or reimbursement, you must pay for the service at the time it’s provided. You will receive the discount off the provider’s usual and customary fees when you pay.
